All Returning Students must submit the following documents and a $25.00 re-application fee (non refundable), prior to scheduling, by mail, in person, and/or emailed to:

  1. Completed Returning Student Admissions Application; *
  2. $25 re-application fee;
  3. Copy of current Immunization record or Notarized New York State Medical Exemption Form (Religious exemptions are no longer accepted, as per NYS Dept of Health);
  4. Copy of Child/Adolescent Health Examination Form; *
