1. Completed Returning Student Admissions Application 

Prior to Class Scheduling in August, the following documents must be submitted to Mrs. Santiago at [email protected] (If Any Updates)

1. Copy of current Immunization Record or Notarized New York State Medical Exemption
2. Copy of Child/Adolescent Health Examination Form
